To Wolf Pup cosmetic is available only to Diablo 4 beta players that have reached level 20. If you’re not one of them, there is no way you can obtain Wolf Pup unless Blizzard changes its mind. If you’ve played beta, follow these steps to equip Wolf Pup in Diablo IV.

And that’s all you need to do to have your loveable pup sleeping on your back while you hack and slash your way to Lilith. My only fear is that all those loud noises will wake my sweet pet. This is the way in Sanctuary!
